The PCP engages with hospitalists for hospitalized patients, performs assessment, diagnosis, treatment, and education, and coordinates health promotion and documentation for complex chronic needs. They lead a care team (medical assistant, facilitator, coordinator) for office patients and collaborate with transitional care teams for non‑office patients. The PCP has an active role in center management, covers for other providers, and recruits patients and additional providers, performing other duties as directed.
