Overview:
The Patient Care Coordinator serves as the primary point of contact for patients receiving behavioral health services in an outpatient setting. This role ensures a seamless, compassionate, and efficient care experience by coordinating patient intake, scheduling, insurance verification, treatment planning support, and follow-up care. Working closely with clinicians, psychiatrists, and administrative staff, the Patient Care Coordinator helps facilitate communication between patients and the care team, maintains accurate clinical and administrative records, and supports the overall delivery of high-quality, patient-centered behavioral health care.
